Your Medical Benefits
Eligibility:
Employees who work a minimum of 30 hours per week are eligible to enroll on their date of hire. You may also enroll your eligible dependents, and they are defined as:
- All Legal Spouses
- Qualified Domestic Partners (same or opposite sex)
- Dependent Children up to age 26 (natural, adopted, foster or stepchildren)
If your dependent child is over age 26 and mentally or physically disabled, coverage can be continued with proof of the ongoing disability.

Anthem Classic PPO
With the PPO plan, you can visit any licensed doctor, hospital or medical provider you want. However, the method of payment and the cost of the service may vary, depending on whether the doctor is in the Anthem network or not. With a PPO, you will be responsible for copays for certain services and will need to meet your deductible and coinsurance for other services.

The Anthem Blue Cross DPPO plan covers a wide range of dental services. You have the freedom to go in or out of network for care; however you can maximize your benefits and pay less out-of-pocket when you see a dentist in the PPO network.
